What PHP Is and the Clinical Problem It Solves
The gap between inpatient treatment and weekly outpatient therapy is enormous. Inpatient and residential treatment provide 24-hour monitoring, separation from triggers, and daily programming. Weekly outpatient therapy provides one session per week. For many adults with moderate to severe addiction or mental health conditions, that is not enough clinical contact to create stability.
PHP closes that gap. At five days per week and six hours per day, PHP provides about 30 hours of clinical programming each week while allowing clients to sleep at home and avoid the cost and disruption of residential treatment. For adults who do not require 24-hour medical supervision, PHP can provide the intensity needed to stabilize symptoms and build a strong step-down plan.

In those situations, detox, inpatient care, or another placement may be the safer starting point.

PHP Schedule and Daily Structure
PHP at Hope Harbor Wellness runs Monday through Friday from 9:00am to 3:00pm. A typical PHP day may include:
- Morning check-in and wellness assessment
- Evidence-based group therapy using CBT and DBT
- Psychoeducation on addiction, relapse prevention, mood disorders, trauma, and recovery skills
- Individual therapy sessions with a licensed clinician
- Psychiatric access for medication review and symptom monitoring
- Family support or family education when appropriate
- Afternoon processing groups and planning for the evening outside treatment
FMLA documentation is available for clients whose work situation requires medical documentation during PHP participation.
Clinical Components of PHP at Hope Harbor Wellness
Integrated Dual Diagnosis Treatment
Every PHP client receives a psychiatric evaluation at the beginning of treatment. Depression, anxiety, PTSD, bipolar disorder, ADHD, and other co-occurring mental health conditions are treated alongside addiction from day one. This is the clinical standard and consistently produces better outcomes than treating addiction and mental health separately.
Cognitive Behavioral Therapy and Dialectical Behavior Therapy
CBT helps clients identify the thoughts, beliefs, and patterns that drive substance use, depression, anxiety, and relapse. DBT helps with emotional regulation, distress tolerance, impulsivity, interpersonal instability, and chronic overwhelm. Together, they form a strong foundation for PHP-level treatment.
EMDR for Trauma
EMDR is available within PHP for clients with trauma histories or PTSD. For many people, trauma is not separate from the addiction or mental health symptoms. It is one of the forces that sustains them. Treating trauma within the PHP framework can be an essential part of making recovery sustainable.
Medication-Assisted Treatment Within PHP
Clients who require MAT, including buprenorphine, naltrexone, or other appropriate medications, can have those services managed within PHP. Medication support is integrated into the treatment framework rather than managed in a separate silo.
PHP vs IOP, Making the Right Choice
PHP is usually the better fit when symptom severity is higher, daily structure is needed, relapse risk is significant, or recent crisis stabilization has occurred. IOP is usually the better fit for moderate severity, step-down care, or adults who are stable enough to need less weekly structure.

How Long PHP Usually Lasts
PHP length depends on progress, symptom severity, relapse risk, and how much structure is still needed. Many people participate for several weeks and then step down into IOP, followed by outpatient care as stability improves.
The goal is not to rush through programming. The goal is to build enough stability that stepping down is actually safe and sustainable.
Does PHP Include Detox
No. Detox and PHP serve different purposes. Detox focuses on stabilization and withdrawal management. PHP focuses on intensive therapy, psychiatric support, relapse prevention, and daily clinical structure once the person is medically stable.
Some people can begin PHP without detox. Others need detox first. That decision depends on withdrawal risk, current substance use, and the clinical assessment.
Sober Living and Supportive Environment
PHP works best when the environment outside treatment supports recovery. Some clients live at home. Others benefit from a more recovery-focused living situation while attending PHP.
