What Is IOP and Who It Is For
Intensive Outpatient Program is the level of care between standard weekly outpatient therapy and Partial Hospitalization Program. IOP provides structure, accountability, and clinical programming several days each week while you continue to live at home and stay engaged in daily life.
IOP is clinically appropriate for adults who need more support than weekly therapy can provide, but who do not require the full-day structure of PHP or inpatient treatment. It is also the most common step-down level after PHP, helping protect progress while clients reintegrate into work, school, and home responsibilities.

Who May Need a Different Level of Care Than IOP
IOP is not the best starting point for every situation. Some people need detox first. Others need the higher daily structure of PHP. The right level of care depends on withdrawal risk, relapse history, mental health stability, and what the home environment looks like right now.
You may need a higher level of care than IOP if you are at risk for medically significant withdrawal, have a home environment that is unstable or actively triggering, need more daily clinical oversight, or are experiencing serious mental health instability. In those situations, starting with detox or PHP may be the safest choice. We determine that through assessment, not guesswork.
IOP Schedule and Structure at Hope Harbor Wellness
IOP at Hope Harbor Wellness runs three to five days per week, with sessions lasting about three hours each. Two scheduling tracks are available.
- Morning IOP: 9:00am to 12:00pm, for clients who have afternoon availability, caregiving responsibilities later in the day, or work evening shifts.
- Evening IOP: 5:30pm to 8:30pm, designed for employed adults who cannot miss work during the day.
Each IOP session includes a combination of group therapy, individual therapy, psychoeducation, skills development, relapse prevention work, and peer support. Individual sessions are part of the IOP framework, not something clients have to arrange separately. The clinical team reviews progress regularly and adjusts the treatment plan as needed.
What the Clinical Programming Covers
Cognitive Behavioral Therapy
CBT is one of the most extensively researched treatments for substance use disorders and many mental health conditions. In IOP, CBT helps clients identify the thoughts, distortions, habits, and triggers that drive substance use or emotional instability, then build practical strategies to interrupt those patterns before they become relapse.
Dialectical Behavior Therapy
DBT is especially helpful for emotional dysregulation, impulsivity, chronic distress, self-harm history, and relationship instability. DBT skills work in IOP focuses on distress tolerance, emotion regulation, interpersonal effectiveness, and mindfulness.
Dual Diagnosis Treatment
Many adults in IOP are dealing with both addiction and mental health symptoms. At Hope Harbor Wellness, psychiatric evaluation happens at intake. Depression, anxiety, PTSD, bipolar disorder, ADHD, and other concerns are assessed alongside substance use, not months later. Treating both from the start produces better outcomes than treating them one at a time.
EMDR for Trauma
For clients with trauma histories or PTSD, EMDR can be integrated into individual therapy within the IOP framework. For many people, trauma is not a side issue. It is one of the central drivers of continued substance use, emotional dysregulation, or relapse.
Medication-Assisted Treatment Within IOP
Clients who need MAT, including buprenorphine for opioid use disorder or naltrexone for alcohol or opioid use disorder, can have those services managed within the IOP framework. This integrated model improves continuity because the same team is coordinating both therapy and medication support.

How Long IOP Usually Lasts
IOP length depends on clinical need, progress, relapse risk, and how much structure the person still needs. Many people stay in IOP for several weeks, often around two to three months, though treatment may be shorter or longer based on the individual.
Length of care is shaped by things like substance use history, mental health needs, consistency in treatment, home stability, and progress toward recovery goals. Many clients eventually step down from IOP into standard outpatient care for continued support.
Does IOP Include Detox
No. Detox and IOP serve different purposes. Detox focuses on stabilization and withdrawal management. IOP focuses on therapy, coping skills, relapse prevention, psychiatric support, and accountability while living at home.
If detox is needed first, we help guide that process and make sure there is a clear next step into treatment once the person is medically stable.

A clinical team member completes a brief intake assessment, usually about 15 to 20 minutes, covering substance use history, mental health history, current medications, insurance, and scheduling needs.
