How Addiction Develops in Veterans – The Clinical Reality

Many veterans face unique risk factors for substance use, including trauma exposure, chronic pain, sleep disruption, PTSD symptoms, and difficulty transitioning back into civilian life. These are specific, identifiable experiences that treatment must address directly:

Combat PTSD and opioid self-medication: Hyperarousal is one of the most debilitating features of combat PTSD, the inability to downregulate a threat-detection system that was calibrated for a combat environment and never received the clinical signal that the threat has ended. Some veterans first notice that opioids temporarily dull both physical pain and emotional distress, especially when they were originally prescribed after an injury. Over time, that short-term relief can develop into opioid dependence or opioid use disorder.

Alcohol and sleep: Combat-related PTSD disrupts sleep dramatically, including nightmares, hyperarousal, and difficulty achieving deep sleep without assistance. Alcohol is a short-term sleep inducer that ultimately disrupts sleep architecture severely with chronic use. The pattern of drinking to sleep, sleeping poorly, and drinking more to compensate is one of the most common veteran alcohol presentations we treat.

Moral injury: The psychological injury resulting from participation in or witnessing events that violate deeply held moral beliefs, a dimension of military trauma that PTSD diagnostic frameworks do not fully capture. Moral injury can produce profound guilt, shame, spiritual disruption, and loss of meaning that may drive self-medication. Effective treatment for veteran addiction that is driven by moral injury requires more than general support. It requires treatment that addresses the moral and emotional dimension of trauma, which EMDR may help address through specific protocols.

TBI and pain: Traumatic brain injury from blast exposure can produce chronic pain, cognitive symptoms, mood instability, and sleep disruption, all of which may contribute to substance use and complicate addiction treatment.
