If you are researching trauma therapy options, you may have encountered both EMDR (Eye Movement Desensitization and Reprocessing) and ART (Accelerated Resolution Therapy). Both use eye movements. Both treat PTSD and trauma-related conditions without requiring detailed verbal description of the traumatic event. But they work differently, produce results on different timelines, and have somewhat different clinical applications.

Both are offered at Hope Harbor Wellness. Here is what you need to know.

How EMDR Works

EMDR is based on the Adaptive Information Processing model – the theory that traumatic memories become “stuck” in the nervous system in a raw, unprocessed state that causes them to remain emotionally activating when triggered. EMDR uses bilateral stimulation (typically guided eye movements, though audio or tactile stimulation can also be used) while the client briefly attends to the traumatic memory.

Decision Guide: What to Ask Before Choosing Care

Question Why it matters What to do next
Are symptoms occasional, daily, or crisis-level? Frequency and severity help determine outpatient therapy, IOP, PHP, or referral needs Complete a clinical assessment
Is safety a concern? Active crisis symptoms may require emergency or inpatient care first Call 988, 911, or go to the ER if immediate safety is at risk
Has weekly therapy been enough? If not, a more structured level of care may be appropriate Ask about IOP or PHP
Are there co-occurring conditions? Trauma, substance use, mood symptoms, and anxiety can change the treatment plan Request an integrated assessment

This process appears to activate the brain’s natural memory reconsolidation mechanisms, allowing the stuck memory to be updated and integrated with the rest of memory – so it becomes a past event rather than a present threat.

EMDR moves through 8 structured phases:

  1. History-taking and treatment planning
  2. Preparation and stabilization
  3. Assessment of target memory
  4. Desensitization via bilateral stimulation
  5. Installation of positive beliefs
  6. Body scan
  7. Closure
  8. Re-evaluation

EMDR is one of the most extensively researched trauma therapies. It is endorsed by the WHO, APA, and VA/DoD. Research consistently shows it is effective for PTSD from any cause.

How ART Works

Accelerated Resolution Therapy was developed by Laney Rosenzweig and is based partly on EMDR but incorporates additional techniques. Like EMDR, ART uses bilateral eye movements while the client holds a distressing image or memory in mind. But ART adds a technique called voluntary image replacement – the client is guided to replace the distressing image associated with the trauma with a neutral or positive image of their choosing.

This active replacement component often produces faster results than EMDR because it does not require the extensive desensitization phase – the client actively transforms the image rather than waiting for the emotional intensity to naturally reduce through processing.

Key features of ART:

  • Typically 1-5 sessions for single-incident trauma
  • Client maintains more active control over the imagery
  • Particularly effective for somatic (body-based) trauma responses
  • Does not require verbal description of the trauma
  • Effective for specific-incident trauma, phobias, grief, and performance anxiety

EMDR vs ART: Side-by-Side Comparison

EMDR ART
Technique Bilateral stimulation while attending to memory Bilateral stimulation + voluntary image replacement
Sessions needed 6-12+ for single-incident PTSD Often 1-5 sessions
Client role Largely passive; let the brain process More active; client replaces imagery
Verbal disclosure Not required Not required
Research base Extensive; 30+ years Growing; promising results
Best for PTSD from any cause; complex trauma Specific-incident trauma; somatic responses; faster timeline
Endorsements WHO, APA, VA/DoD SAMHSA, military and veteran organizations

Which Should You Choose?

There is no universal answer – the right choice depends on your trauma history, clinical presentation, timeline, and the availability of trained therapists.

Consider EMDR if:

  • You have complex or developmental trauma
  • You prefer a more extensively researched approach
  • You have time for a longer treatment course

Consider ART if:

  • Your trauma is tied to specific incidents
  • You want faster results
  • You have somatic responses to trauma (physical symptoms triggered by trauma reminders)
  • You prefer more active participation in the imagery work

In practice: A trained clinician at Hope Harbor Wellness can assess your presentation and recommend the most appropriate trauma approach – or integrate both. Call 770-573-9546.
